Your Initial Visit to Dr. Garber's Office

1. All of your medical insurance cards, both primary and secondary
2. Your co-payment (cash, check, or credit card)
3. For Keystone and Aetna HMO patients: you must have your primary care physician issue a referral into the appropriate computer system; hand-written referrals are no longer valid.
4. A complete list of your medications
5. Any pertinent medical records, biopsy reports, and X-rays
6. The name and address of your referring physician
7. You can download, print and fill out the following forms, which will be needed for your first visit:
